Physical Therapist (PT)
Location- Maustin, WI
Pay Range - $91,000 to $120,000 Yearly
Full-Time
The Physical Therapist provides patient-centered care focused on pain management, improving mobility and functional independence, reducing fall risk, and supporting safe discharge planning. This role helps patients restore movement, recover from injury or surgery, and improve overall quality of life through evidence-based physical therapy interventions.
Evaluate and treat patients with movement dysfunction, pain, or physical impairments.
Develop and implement individualized treatment plans to improve functional mobility and independence.
Assist patients in recovery following surgery or illness.
Reduce risk of adverse events such as falls, deep vein thrombosis (DVT), and mobility-related complications.
Support patients in returning to daily activities and work-related tasks as appropriate.
Accurately document all aspects of patient care, including evaluations, daily treatment notes, progress reports, and discharge summaries.
Complete documentation related to insurance approvals, denials, and required communications.
Communicate effectively with healthcare providers, insurance companies, patients, rehabilitation staff, and supervisors via phone and electronic systems.
Assist with maintaining a clean and safe treatment environment, including treatment rooms and shared therapy areas.
Support departmental operations such as assisting with laundry and general clinic upkeep.
Perform additional duties as assigned to support patient care and departmental efficiency.
Graduate of an accredited Physical Therapy program required.
Eligible for Physical Therapist licensure in the practicing state.
Active, unrestricted Physical Therapist license required.
Prior clinical experience preferred.
Experience in a healthcare or rehabilitation setting is a plus.
Strong clinical assessment and treatment planning skills.
Excellent documentation accuracy and attention to detail.
Effective verbal and written communication skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Strong organizational, time-management, and problem-solving abilities.
Commitment to maintaining a safe, clean, and patient-focused clinical environment.
