Registered Nurse – Case Manager

Location - Juneau, AK

Pay Range - 83,000 to $100,000 Yearly


Job Summary:

The Registered Nurse Case Manager coordinates the delivery of timely, appropriate, and cost-effective patient care. This role focuses on care coordination, patient advocacy, interdisciplinary collaboration, and chronic condition management to support optimal health outcomes across diverse patient populations.


Schedule:

  • Monday–Friday

  • Day shift (standard business hours)


Key Responsibilities:

Care Coordination & Case Management:

  • Actively manage assigned patient panels, including acute, chronic, and preventive care needs.

  • Collaborate with medical providers and interdisciplinary teams to establish individualized care plans and patient-centered goals.

  • Coordinate multidisciplinary care conferences for high-risk or complex patients.

  • Monitor clinical reminders and care gaps through electronic health records and provide outreach and patient education.

  • Participate in regular case management reviews to support patients with complex care needs.

  • Advocate for patients and families to support informed decision-making, independence, and self-care.


Referral & Specialty Coordination:

  • Coordinate referrals to specialty and external services, ensuring completeness of records and documentation.

  • Track referral progress and outcomes, providing status updates to providers as needed.

  • Identify and resolve barriers or delays in care delivery.

  • Serve as a liaison among case managers, discharge coordinators, providers, and specialty services.


Clinical & Nursing Duties:

  • Perform staff nursing responsibilities including patient assessment, documentation, education, and medication administration.

  • Support specialty clinics by preparing clinical spaces, assisting with procedures, and providing clinical support during visits.

  • Provide care coordination for specialty populations, including obstetric patients, ensuring continuity of care across the care continuum.

  • Support telehealth and diagnostic coordination as required.


Teamwork & Professional Practice:

  • Collaborate effectively with clinical and administrative teams.

  • Participate in department initiatives, quality improvement activities, and development of case management practices.

  • Serve as an active member of the clinical team and provide leadership support when appropriate.

  • Participate in triage and emergency response activities as required.

  • Maintain annual competencies and adhere to safety, infection control, and professional standards.


Education & Licensure:

  • Graduate of an accredited school of nursing; Bachelor’s degree preferred.

  • Current, unrestricted Registered Nurse (RN) license in any U.S. state.

  • Basic Life Support (BLS) certification.

  • Clinical competency validation within required timeframes.

  • High school diploma or equivalent.


Experience Requirements:

  • Minimum of 1–2 years of general nursing experience in an ambulatory, outpatient, or case management setting.

  • Case management experience preferred.


Knowledge & Skills:

  • Strong understanding of the nursing process and evidence-based practice.

  • Knowledge of care coordination, patient advocacy, and chronic disease management.

  • Proficiency in electronic health records and clinical documentation.

  • Strong assessment, communication, and organizational skills.

  • Ability to work independently in a fast-paced environment while collaborating within multidisciplinary teams.

  • Effective problem-solving, conflict resolution, and critical thinking abilities.


Physical Requirements:

  • Frequent standing, walking, sitting, and use of hands and arms.

  • Occasional bending, stooping, kneeling, crouching, climbing, or crawling.

  • Ability to lift and/or move up to 50 lbs.


Required Certifications:

  • Basic Life Support (BLS)

  • Clinical Competency Assessment

  • Active Registered Nurse License
