Registered Nurse – Case Manager OB/GYN
Juneau, Alaska
Provides case management and clinical support for OB/GYN patients.
Coordinates care across providers, referrals, and specialty clinics to ensure timely, efficient, and high-quality outcomes.
Acts as the primary clinical contact for patient care coordination within the OB/GYN department.
Actively manage OB/GYN patients and collaborate with medical providers and interdisciplinary teams to establish individualized care goals.
Perform staff nursing duties including assessment, documentation, education, and administration of medications and therapies.
Coordinate patient referrals to Surgery/Specialty Clinics, reviewing records to ensure completeness.
Track referral progress and outcomes, providing periodic status reports to providers.
Identify delays and obstacles to care, proactively addressing issues to improve the patient care process.
Build rapport and collaborate effectively with clinical staff, departments, and interdisciplinary teams.
Participate in clinic and department improvement efforts.
Perform other duties or special projects as assigned to provide comprehensive healthcare services.
Current, full, unrestricted Registered Nurse (RN) license in Alaska or other U.S. state.
Bachelor’s degree in Nursing preferred.
Basic Life Support (BLS) certification.
Clinical Competency required within 3 months of hire and every three years.
High school diploma or equivalent.
Knowledge of the nursing process and ability to apply it in a clinical setting.
Familiarity with customer service principles and infection control standards.
Ability to work in a fast-paced, multidisciplinary healthcare environment.
Patient assessment, anticipation of needs, and data collection.
Oral and written communication.
Critical thinking and problem-solving.
Ability to multi-task and work independently while being a collaborative team member.
Willingness to teach and seek assistance when encountering new or challenging situations.
Practice evidence-based nursing and shared governance principles.
Regular talking, hearing, standing, walking, sitting, and use of hands/arms.
Occasional climbing, balancing, stooping, kneeling, crouching, or crawling.
Must lift and/or move up to 50 lbs.
