Position Title: Advanced Clinical Pharmacist (ACP)

Desired Location: Lake City, Florida (Onsite)


Shift Details

  • Night Shift (7 on / 7 off)

  • Rotating Weekends

  • Shift Hours: Wednesday–Tuesday | 6:45 PM – 7:15 AM


Position Summary

The Advanced Clinical Pharmacist (ACP) supports critical care services by coordinating, developing, and maintaining clinical pharmacy programs to ensure safe, effective, and evidence-based medication use. This role works closely with interdisciplinary clinical teams to optimize patient outcomes while ensuring compliance with pharmacy standards, policies, and regulatory requirements.


Key Responsibilities

  • Review medication orders and pharmacy technician work to ensure accuracy, appropriateness, dosing, route, and potential drug interactions

  • Adjust medication dosages and schedules based on patient condition and established policies

  • Ensure safe medication dispensing in accordance with age-specific guidelines

  • Prepare and oversee sterile IV admixtures in compliance with USP 797 standards

  • Participate in clinical data evaluation, medication-use analysis, and drug/drug-class monograph development

  • Support development and maintenance of clinical pharmacy toolkits

  • Provide oversight of pharmacy distribution processes, including order receipt, storage, dispensing, and controlled substance handling

  • Monitor and reconcile controlled substance transactions and maintain compliance with DEA documentation

  • Assist with physician consults and provide accurate, timely drug information

  • Communicate effectively with physicians, nursing staff, patients, and families in a professional and empathetic manner

  • Promote a patient-centered environment that supports quality care and safety


Required Qualifications

Education

  • Graduate of an ACPE-accredited College of Pharmacy

  • Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD)


Experience

  • Open to experience level

  • New graduates accepted with active Florida Pharmacist License

  • Hospital experience or completion of a pharmacy residency preferred but not required

  • Candidates should demonstrate familiarity with clinical resources used to maintain current clinical knowledge


Licensure & Skills

  • Active Florida Pharmacist License – Required

  • Strong customer service and communication skills

  • Critical thinking and sound clinical judgment

  • Ability to work independently in a fast-paced clinical environment
