Location: Bronx, NY 10461
Contract: 13 Weeks
Schedule: 10:00 AM – 6:00 PM | 40 Hours

Role Description
The Emergency Care Coordinator provides care coordination, utilization review, and discharge planning to support timely and safe patient transitions from the ED.

Responsibilities

Coordinate patient care in the ED
Conduct utilization management reviews
Plan and implement discharge processes
Identify and resolve discharge barriers
Ensure regulatory compliance and documentation

Requirements

Active NY RN license
5 years clinical RN experience
Background in care coordination and discharge planning
ASN/ADN
BLS certification

Preferred

BSN or higher
Case Management certification
