Overview
The RN Discharge Planning Coordinator supports safe transitions of care from the Emergency Department through care coordination and utilization review.

Responsibilities

Lead discharge planning efforts for ED patients
Perform utilization management reviews
Coordinate post-acute care resources
Partner with providers to reduce length of stay
Maintain accurate care management documentation

Requirements

Active NY RN license
5+ years RN experience
Experience with discharge planning and care coordination
ASN/ADN required
BLS certification

Preferred

BSN/MSN
Case Management certification
