Location: Oxford, New York
We are seeking a Licensed Practical Nurse (LPN) to support a Long Term Care (LTC) setting. The LPN will provide direct nursing care under the supervision of a Registered Nurse, physician, or other licensed healthcare provider. This role focuses on maintaining resident health, administering treatments, and supporting individualized care plans in a structured care environment.
Provide nursing care consistent with established treatment plans and scope of practice.
Assist Registered Nurses, physicians, and licensed healthcare providers in identifying resident needs and developing care plans.
Administer prescribed medications and perform nursing procedures according to facility policies.
Obtain and accurately record vital signs, including temperature, blood pressure, pulse, and respirations.
Observe, monitor, and document residents’ physical and behavioral conditions.
Identify and report changes in health status or responses to medications promptly to supervising clinicians.
Support resident comfort, safety, and well-being while promoting dignity and quality of life.
Maintain accurate clinical documentation in accordance with regulatory and facility standards.
Adhere to infection control, safety, and compliance guidelines at all times.
Active Licensed Practical Nurse (LPN) license with current registration in New York State.
Ability to work effectively under the direction of a Registered Nurse or licensed provider.
Strong clinical observation, documentation, and communication skills.
Knowledge of nursing principles, long-term care practices, and patient safety standards.
Ability to follow established procedures and respond appropriately to changes in resident condition.
