Role Overview

We are seeking a Licensed Practical Nurse (LPN) to support a Long Term Care (LTC) setting. The LPN will provide direct nursing care under the supervision of a Registered Nurse, physician, or other licensed healthcare provider. This role focuses on maintaining resident health, administering treatments, and supporting individualized care plans in a structured care environment.


Key Responsibilities

  • Provide nursing care consistent with established treatment plans and scope of practice.

  • Assist Registered Nurses, physicians, and licensed healthcare providers in identifying resident needs and developing care plans.

  • Administer prescribed medications and perform nursing procedures according to facility policies.

  • Obtain and accurately record vital signs, including temperature, blood pressure, pulse, and respirations.

  • Observe, monitor, and document residents’ physical and behavioral conditions.

  • Identify and report changes in health status or responses to medications promptly to supervising clinicians.

  • Support resident comfort, safety, and well-being while promoting dignity and quality of life.

  • Maintain accurate clinical documentation in accordance with regulatory and facility standards.

  • Adhere to infection control, safety, and compliance guidelines at all times.


Qualifications

  • Active Licensed Practical Nurse (LPN) license with current registration in New York State.

  • Ability to work effectively under the direction of a Registered Nurse or licensed provider.

  • Strong clinical observation, documentation, and communication skills.

  • Knowledge of nursing principles, long-term care practices, and patient safety standards.

  • Ability to follow established procedures and respond appropriately to changes in resident condition.
