Job Location: Bronx, NY 10461
Contract Length: 13 weeks
Shift: Day Shift – 10:00 AM to 6:00 PM
Travel: $69 – $72 per hour
Local: $55 – $59 per hour
The Care Manager RN is responsible for coordinating patient care across the continuum, with a strong focus on utilization review, discharge planning, and care coordination. This role ensures safe, efficient, and high-quality patient transitions while collaborating with interdisciplinary teams to support optimal patient outcomes.
Coordinate and manage patient care plans to ensure appropriate utilization of services.
Conduct utilization reviews and ensure compliance with regulatory and payer requirements.
Facilitate discharge planning and safe transitions to the next level of care.
Collaborate with physicians, nursing staff, social workers, and multidisciplinary teams.
Communicate effectively with patients, families, and healthcare providers regarding care plans and discharge needs.
Maintain accurate and timely documentation in the electronic medical record system.
Advocate for patients to ensure access to appropriate services and resources.
Ensure compliance with hospital policies, regulatory standards, and quality measures.
Perform other related duties as assigned.
Active New York State Registered Nurse (RN) license (single state; compact licenses not accepted).
BLS certification (American Heart Association required).
PRI and Screen certification required.
Minimum of 5 years of clinical experience as a Registered Nurse.
Experience with care coordination, utilization review, and discharge planning.
Computer literacy and proficiency with electronic medical records.
Strong communication, organizational, and care coordination skills.
