Job Title: Care Manager – Registered Nurse (RN)

Job Location: Bronx, NY 10461
Contract Length: 13 weeks
Shift: Day Shift – 10:00 AM to 6:00 PM


Pay Rates:

  • Travel: $69 – $72 per hour

  • Local: $55 – $59 per hour


Position Summary:

The Care Manager RN is responsible for coordinating patient care across the continuum, with a strong focus on utilization review, discharge planning, and care coordination. This role ensures safe, efficient, and high-quality patient transitions while collaborating with interdisciplinary teams to support optimal patient outcomes.


Key Responsibilities:

  • Coordinate and manage patient care plans to ensure appropriate utilization of services.

  • Conduct utilization reviews and ensure compliance with regulatory and payer requirements.

  • Facilitate discharge planning and safe transitions to the next level of care.

  • Collaborate with physicians, nursing staff, social workers, and multidisciplinary teams.

  • Communicate effectively with patients, families, and healthcare providers regarding care plans and discharge needs.

  • Maintain accurate and timely documentation in the electronic medical record system.

  • Advocate for patients to ensure access to appropriate services and resources.

  • Ensure compliance with hospital policies, regulatory standards, and quality measures.

  • Perform other related duties as assigned.


Licensure & Certifications:

  • Active New York State Registered Nurse (RN) license (single state; compact licenses not accepted).

  • BLS certification (American Heart Association required).

  • PRI and Screen certification required.


Required Clinical Skills & Experience:

  • Minimum of 5 years of clinical experience as a Registered Nurse.

  • Experience with care coordination, utilization review, and discharge planning.

  • Computer literacy and proficiency with electronic medical records.

  • Strong communication, organizational, and care coordination skills.
