Location: Nashville, TN (Onsite)
Schedule: Full-Time | Day Shift | No Weekends

Compensation & Incentives

  • Salary Range: $109,762 – $135,000 (based on experience)

  • Annual Incentive Bonus: 17.5%

  • Sign-On Bonus: $10,000

  • Relocation Assistance: Available on a case-by-case basis

Role Overview

The Director of Case Management holds full operational accountability for hospital-wide case management services, including utilization management, discharge planning, and care coordination. This leader ensures regulatory compliance while driving measurable improvements in length of stay, patient flow, and resource utilization.

The role partners closely with executive leadership, physicians, finance, and clinical teams to align case management strategy with organizational and financial objectives.

Key Responsibilities

  • Lead and oversee daily operations of hospital-wide Case Management services

  • Ensure compliance with all regulatory requirements, policies, and procedures

  • Direct care coordination and discharge planning to support safe, timely transitions of care

  • Allocate staffing and resources to achieve quality, productivity, and financial targets

  • Analyze operational and clinical data to drive performance improvement and LOS reduction

  • Collaborate closely with:

    • Chief Medical Officer (CMO)

    • Chief Nursing Officer (CNO)

    • Facility CFO

    • Division-level Case Management leadership

    • Ancillary services and post-acute partners

  • Develop, mentor, and sustain high-performing interdisciplinary teams

  • Lead onboarding, competency management, and performance development

  • Drive change management initiatives across multiple stakeholders

  • Maintain executive-level communication, presentation, and leadership presence

Required Qualifications

  • Bachelor’s Degree – required

  • Master’s Degree (Nursing, Health Administration, or Business Administration) – preferred

  • Active licensure (one required):

    • Registered Nurse (RN)

    • Licensed Clinical Social Worker (LCSW)

    • Licensed Master Social Worker (LMSW)

  • 3+ years of recent acute-care hospital Case Management experience

  • 2+ years of Case Management leadership experience

  • Proven success in:

    • Length-of-stay reduction

    • Hospital capacity and patient flow management

    • Leading teams through performance improvement initiatives

    • Data-driven operational decision-making
