Job Title: Licensed Practical Nurse (LPN)

Location: Montour Falls, NY
Contract Type: Long-Term / Long-Term Care
Contract Length: 13 Weeks
Shift: Evening Shift
Schedule: 4 shifts per week | 8 hours per shift
Pay Rate: $37/hour

Position Overview

A long-term care facility is seeking a Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a skilled nursing and long-term care setting. The LPN will work collaboratively with the healthcare team to ensure resident safety, comfort, and overall well-being while maintaining compliance with clinical standards and regulations.

Responsibilities

  • Provide direct nursing care to residents in a skilled nursing or long-term care environment

  • Administer medications and treatments in accordance with physician orders and facility protocols

  • Monitor, assess, document, and report residents’ vital signs and changes in condition

  • Perform wound care and basic medical procedures within scope of practice

  • Assist residents with activities of daily living (ADLs) as needed

  • Maintain accurate, timely, and compliant patient documentation

  • Collaborate effectively with Registered Nurses, physicians, and interdisciplinary care teams

  • Respond promptly and appropriately to emergencies and resident care needs

  • Ensure adherence to facility policies, procedures, and state regulations

  • Promote patient safety, dignity, confidentiality, and comfort

  • Support residents’ physical, emotional, and psychosocial health

Required Qualifications

  • Active Licensed Practical Nurse (LPN/LVN) license in good standing

  • Minimum of 1 year experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting preferred

  • Current BLS/CPR certification

  • Strong communication, teamwork, and documentation skills

  • Ability to work effectively in a fast-paced, team-oriented environment

  • Physical ability to stand, walk, lift, and assist residents as required
