Role Overview: This is a senior leadership role responsible for overseeing enterprise-wide Case Management operations within a large, high-acuity acute care hospital. The Director provides strategic, operational, and clinical leadership across Case Management, Social Work, and Chaplain Services, ensuring regulatory compliance, optimized patient throughput, effective discharge planning, and strong interdisciplinary collaboration.

This role requires an executive presence, strong data-driven decision-making capability, and the ability to independently lead large-scale initiatives across physicians, finance, nursing leadership, and post-acute partners.

Key Responsibilities:

  • Provide overall operational leadership for hospital-wide Case Management functions
  • Direct and evaluate the case management model, staffing plans (FTEs, skill mix), onboarding, competencies, and use of technology
  • Ensure effective care coordination and discharge planning, including timeliness, quality, and documentation compliance
  • Allocate resources to meet productivity, quality, and financial performance goals
  • Partner closely with Finance (CFO) and Division Case Management leadership to achieve strategic objectives
  • Monitor departmental performance through data analysis and implement sustainable improvement initiatives
  • Ensure full compliance with internal policies, SOPs, and regulatory standards
  • Build and maintain strong working relationships with executive leaders (CMO, CNO), physicians, legal, ethics, and ancillary services
  • Lead and develop a high-performing leadership team across multiple disciplines
  • Participate in rotating on-call leadership coverage as needed based on hospital volume

Scope & Complexity:

  • Hospital Size: ~790 beds
  • Trauma Level: II
  • Annual Volumes: ~34,000 inpatient admissions, ~88,000 ED visits, ~215,000 outpatient visits
  • Team Size: ~110 FTEs
  • Direct reports include 4 Managers, 1 Social Work Supervisor, and administrative support
  • Oversight spans Case Management, Social Work, and Chaplaincy

Required Qualifications:

  • Bachelor’s Degree – Required
  • Master’s Degree (Nursing, Healthcare Administration, or Business) – Preferred
  • Active licensure in one of the following: RN, LCSW, LMSW
  • 3+ years of acute care hospital Case Management experience
  • 2+ years of Case Management leadership experience
  • Proven ability to:
    • Independently analyze data and develop action plans
    • Lead large, multidisciplinary teams
    • Build strong physician and post-acute community partnerships
    • Operate effectively in a fast-paced, high-volume environment

Compensation & Incentives:

  • Base Salary Range: $135,004 – $202,555 (experience-based)
  • Annual Incentive Bonus: 17.5%
  • Sign-On Bonus: $10,000
  • Relocation Assistance: Case-by-case
