Job Title: Hospice Registered Nurse (RN) – Home Care

Location: Hyannis, MA (travel required across the entire Cape, including the bridge and as far east as Yarmouth, Harwich, Orleans)

Schedule & Shift:

  • Monday – Friday (occasional weekends & holidays)

  • Primary Shift: 1:00 PM – 9:00 PM

  • Occasional on-call: 9:00 PM – 8:00 AM

Job Summary: The Hospice RN provides skilled nursing care, pain and symptom management, and supportive care to maximize comfort and quality of life for patients and their families. This role involves providing care to patients in both homes and facilities and requires travel throughout the assigned region. The RN is an integral member of the interdisciplinary team.

Primary Duties & Responsibilities:

  1. Participate in interdisciplinary patient case conferences as needed.

  2. Assess patients critically and respond appropriately to changes in condition.

  3. Prioritize patient needs and manage time efficiently.

  4. Demonstrate excellent verbal and written communication skills.

  5. Interact compassionately with patients, families, co-workers, and management.

  6. Provide patient care with a caring and professional attitude.

  7. Notify MDs or other care providers of significant patient condition changes.

  8. Complete admission and other required paperwork timely.

  9. Record narrative documentation reflecting skilled care.

  10. Clarify and obtain valid MD orders for all care delivered.

  11. Ensure admission paperwork completion by 9 AM the next business day.

  12. Adhere to standards set forth by the American Nurses Association and hospice philosophy of care.

  13. Explain the hospice philosophy and scope of care to patients, families, and nursing home staff.

  14. Implement Plan of Care under attending physician direction, complying with federal and state regulations.

  15. Supervise Hospice Aides every 14 days per regulations.

  16. Assist in orientation and mentoring of new staff as requested.

  17. Assume additional responsibilities as assigned by management.

Education / Experience / Training:

  • Current RN license in Massachusetts

  • Successful completion of pre-hire medication test

  • Minimum 2 years Hospice RN Case Management experience

  • Hospice Home Care experience required; IV certification, BSN, and computer skills preferred

Travel Requirement:

  • Must be able to travel to patients’ homes across Hyannis, including the bridge and eastern towns (Yarmouth, Harwich, Orleans).
