Job Description
We are seeking a licensed Physical Therapist (PT) for a contract assignment in a skilled nursing and rehabilitation setting. The Physical Therapist will be responsible for evaluating patients, developing individualized treatment plans, and delivering evidence-based therapy services to improve mobility, strength, balance, and overall functional independence.
This position requires flexibility, as the provider will split time between two affiliated facilities during the workweek.
Perform comprehensive physical therapy evaluations and assessments.
Develop and implement individualized treatment plans based on patient needs and goals.
Provide therapeutic exercises, manual therapy, gait training, and functional mobility interventions.
Monitor patient progress and adjust treatment plans as appropriate.
Educate patients and caregivers on exercises, safety techniques, and mobility strategies.
Maintain accurate and timely documentation in compliance with facility policies and regulatory standards.
Collaborate with interdisciplinary healthcare teams to ensure coordinated, patient-centered care.
Adhere to all state, federal, and professional practice guidelines.
Scheduled across two affiliated facilities
Three days per week at the primary location
Two days per week at a secondary location
Active Physical Therapist (PT) license in the practicing state
Experience in skilled nursing or rehabilitation setting preferred
Strong clinical, documentation, and communication skills
Ability to float between facilities as required
