Role Overview
The facility is seeking one experienced Wound Care Nurse (either RN or LPN) to perform advanced wound management, prevention, patient education, and interdisciplinary collaboration in a long-term care/skilled nursing setting.
Perform wound assessments, reassessments, and ongoing wound management for all residents in accordance with policy.
Develop and implement skin care programs to prevent skin breakdown.
Participate in the development and enhancement of policies for wound care, wound prevention, ostomy care, fistula/tube management, and incontinence.
Revise care plans based on resident response to treatment and evaluate overall treatment effectiveness.
Assess and reassess pain; implement appropriate pain management strategies.
Educate residents and families regarding pain management and wound care processes.
Serve as a resource for nursing staff, providing in-service training on wound prevention, wound care, ostomy care, and incontinence.
Communicate effectively with nursing staff and physicians to ensure continuity of care.
Collaborate with interdisciplinary healthcare professionals as appropriate.
Ensure documentation meets all current standards, regulatory requirements, and best practices.
Attend ongoing training and continuing education related to wound prevention, wound management, and ostomy care to stay current with advancements.
Active, unrestricted Arizona RN or LPN license.
CWOCN (Wound, Ostomy, and Continence Nurse) or CWCN (Certified Wound Care Nurse) required.
Eligible for and able to maintain an Arizona Level 1 Fingerprint Clearance Card.
Current CPR card (must include physical assessment component).
Prior experience in skilled nursing or long-term care environments.
