• Candidate must have an active account with the Delaware DHSS Background Check Center.

  • This is the only approved platform for mandatory background checks and fingerprinting.

  • Agencies must have an active Background Check Center account to submit any candidate in DE.

Position Overview

The Licensed Practical Nurse (LPN) will provide high-quality patient care in a skilled nursing facility setting. This role requires strong clinical experience, reliability, and the ability to work collaboratively within the care team. Candidates must be comfortable working in a fast-paced environment and providing compassionate, patient-centered care.

Key Responsibilities

• Deliver direct nursing care to residents under RN and physician supervision.

• Administer medications and treatments per facility protocols.

• Monitor patient conditions, document changes, and report concerns to supervising staff.

• Support care plans, safety standards, and infection control protocols.

• Provide patient education, emotional support, and assistance with daily living activities.

• Collaborate with interdisciplinary staff to ensure continuity of care.

• Maintain accurate, timely charting and clinical documentation.

Requirements

• Active LPN license in good standing.

• Active DE business license required.

• Active DHSS Background Check Center account required.

• Local candidates only.

• Ability to work every other weekend (E/O weekend).

• Previous experience in long-term care or skilled nursing preferred.
