Location: Salinas, CA
The Speech-Language Pathologist (SLP) will assess, diagnose, and treat residents with speech, language, cognitive-communication, and swallowing disorders in a skilled nursing environment. The clinician will collaborate with interdisciplinary team members to develop and implement effective treatment plans that enhance functional communication and improve overall quality of life.
Conduct comprehensive speech, language, cognitive, and swallowing assessments.
Develop individualized treatment plans to address identified deficits and functional goals.
Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia.
Document evaluations, daily notes, progress reports, and discharge summaries in accordance with regulatory and facility standards.
Educate residents, caregivers, and staff on communication strategies and swallowing safety.
Participate in interdisciplinary team meetings and contribute clinical expertise to care planning.
Monitor progress and adjust treatment plans based on patient response and functional outcomes.
Ensure compliance with state licensure requirements and professional practice guidelines.
Support a patient-centered environment focused on functional improvement and safety.
Collaborate closely with nursing, OT, PT, and other rehabilitation team members to ensure coordinated care.
Active and unrestricted SLP license in the state of assignment.
Skilled nursing or geriatric rehab experience preferred.
Strong assessment, treatment planning, and documentation skills.
Excellent communication and clinical judgment abilities.
