Compensation: $109,762–$135,000 annually
Incentive Bonus: 17.5%
Sign-On Bonus: $10,000

"Navitas Healthcare, LLC" is seeking an experienced and strategic Director of Case Management to lead a hospital-wide case management program within a high-volume, Level I Trauma setting. This leadership role oversees all aspects of case management operations, including care coordination, discharge planning, utilization management, and interdisciplinary collaboration.
The Director is responsible for driving performance outcomes, improving patient throughput, reducing length of stay, optimizing resource utilization, and ensuring compliance with organizational and regulatory standards. This position requires strong operational leadership, data-driven decision-making, and the ability to influence at all levels of the organization.
Bachelor’s degree required; Master’s in Nursing, Health Administration, or Business Administration preferred.
Active licensure as an RN, LCSW, or LMSW required.
Minimum 3 years of recent acute care hospital case management experience.
Minimum 2 years of case management leadership experience.
Proven success in reducing length of stay, improving throughput, and leading high-performance teams.
Strong analytical, process improvement, and change-management skills.
Demonstrated ability to navigate complex stakeholder relationships and achieve organizational outcomes.
Lead and manage departmental operations, including staffing strategies, onboarding, competencies, and workflow optimization.
Ensure optimal resource utilization, financial stewardship, and achievement of productivity and quality benchmarks.
Oversee care coordination, discharge planning, utilization review, and transition-of-care processes.
Ensure accurate, timely, and compliant documentation across case management workflows.
Collaborate closely with executive leadership to support organizational initiatives and operational goals.
Monitor departmental performance using data-driven metrics and implement sustainable improvement initiatives.
Maintain compliance with internal policies, regulatory standards, and corporate procedures.
Foster strong relationships with clinical teams, ancillary departments, Ethics & Compliance, Legal, and other stakeholders.
Serve as a subject matter expert and change leader for case management practices.
Demonstrate executive presence and provide clear, effective communication in leadership forums.
Uphold organizational mission, values, and Code of Conduct.
