Shift: Day Shift (8:00 AM – 4:30 PM, 30-minute meal break)
Position Overview
A skilled and compassionate Speech-Language Pathologist (SLP) is needed for a 13-week contract assignment at a rehabilitation and long-term care facility. The SLP will provide high-quality therapy services to residents, focusing on communication, cognition, and swallowing function to support overall quality of life.
Conduct speech, language, cognitive, and swallowing evaluations.
Develop individualized treatment plans based on clinical findings.
Provide therapeutic interventions to improve communication and swallowing abilities.
Document evaluations, daily notes, progress reports, and discharge summaries according to facility and regulatory standards.
Collaborate with interdisciplinary teams, including nursing, PT, OT, and social services.
Participate in care planning meetings and family education when applicable.
Maintain compliance with facility policies, state regulations, and ASHA guidelines.
Active state license as a Speech-Language Pathologist.
Certificate of Clinical Competence (CCC-SLP) preferred.
Previous experience in skilled nursing or rehab settings strongly preferred.
Strong communication, assessment, and documentation skills.
Ability to work independently and as part of a multidisciplinary team.
