The Speech-Language Pathologist (SLP) will provide evaluation and treatment services to residents with speech, language, cognitive-communication, and swallowing disorders within a skilled nursing environment. This role supports improved functional outcomes and quality of life for residents.
Conduct initial evaluations and develop individualized treatment plans.
Provide therapy for speech, language, cognition, voice, and swallowing.
Maintain timely and accurate documentation per facility and regulatory standards.
Participate in interdisciplinary team meetings and care planning.
Educate residents, families, and staff on communication and swallowing strategies.
Ensure therapy practices meet compliance and ethical standards.
Travel/split time between designated facilities as needed.
Active state license as a Speech-Language Pathologist (SLP).
Master’s degree in Speech-Language Pathology.
Experience in SNF/long-term care preferred.
Ability to work a full-time schedule with weekend availability.
Flexibility to work between multiple sites.
Strong clinical documentation and communication skills.
