The Speech-Language Pathologist (SLP) will provide comprehensive therapy services to patients within a skilled nursing/rehabilitation environment. The role involves assessing communication and swallowing disorders, creating individualized treatment plans, and delivering high-quality, patient-centered therapy.
Conduct evaluations of speech, language, cognitive-communication, and swallowing disorders.
Develop and implement individualized treatment plans based on patient needs and clinical best practices.
Deliver therapy sessions focused on functional communication, swallowing safety, and cognitive rehabilitation.
Maintain accurate, timely, and compliant documentation of assessments, progress notes, and discharge summaries.
Collaborate with interdisciplinary team members including PT, OT, nursing, and case management.
Educate patients, caregivers, and staff on therapy goals and strategies to support functional improvement.
Participate in care plan meetings and contribute to overall patient management discussions.
Follow all regulatory, professional, and ethical standards.
Active and unrestricted SLP license in the state of assignment.
Master’s degree in Speech-Language Pathology.
Certificate of Clinical Competence (CCC-SLP) preferred.
Previous SNF or rehab experience strongly preferred.
Strong communication, documentation, and interpersonal skills.
