Shift: Day Shift (Full-Time)
We are seeking an experienced and dedicated Physical Therapist (PT) to join a rehabilitation-focused healthcare environment. The PT will assess patient needs, develop individualized treatment plans, and provide evidence-based therapy to help patients restore mobility, manage pain, and improve overall function.
Conduct detailed physical therapy evaluations and functional assessments.
Create and implement individualized treatment plans tailored to patient goals and clinical status.
Provide therapeutic exercises, manual therapy, mobility training, and other modalities as appropriate.
Monitor patient progress and adjust treatment plans as needed.
Accurately document treatment sessions, progress notes, and care plans according to regulatory standards.
Collaborate with interdisciplinary care teams including OT, SLP, nursing, and physicians.
Educate patients and caregivers on home exercise programs, safety techniques, and rehabilitation goals.
Participate in care conferences and facility meetings as required.
Active and unrestricted Physical Therapist (PT) license in the state of assignment.
Prior experience in a skilled nursing facility, rehab center, or similar patient care setting (preferred).
Strong clinical assessment, therapeutic planning, and documentation skills.
Excellent communication and patient-engagement abilities.
Full-time schedule; day shift coverage.
35-hour weekly guarantee.
Facility will provide necessary orientation and onboarding.
