Job Title: Licensed Practical Nurse (LPN)

Shift: 5:00 AM – 12:30 PM
Pay Rate: $33/hr
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team on a contract basis in Phoenix, AZ. The LPN will provide high-quality patient care under the supervision of a Registered Nurse (RN) or physician while ensuring adherence to state regulations and organizational standards.
Provide direct patient care under RN or physician supervision.
Monitor and document vital signs such as temperature, pulse, respiration, and blood pressure.
Administer medications and treatments, including injections and wound care, in accordance with state guidelines.
Assist with patient assessments and promptly report changes in condition to the supervising clinician.
Support patients with Activities of Daily Living (ADLs), including bathing, grooming, feeding, toileting, and mobility.
Perform wound care, dressing changes, and catheter care as assigned.
Collect laboratory specimens (urine, stool, and blood—if permitted by state regulations).
Accurately document patient care in the Electronic Medical Record (EMR).
Collaborate with RNs, physicians, and interdisciplinary teams to support care plan development and execution.
Maintain a clean, safe, and compliant healthcare environment following infection control and safety protocols.
Provide patient and family education on care plans, medications, and discharge instructions when appropriate.
Assist with admissions, discharges, and patient transfers.
Offer emotional support to patients and families to promote a positive care experience.
Minimum of 2 years of recent LPN experience.
Active Arizona LPN license.
Current BLS certification through the American Heart Association (AHA).
