Job Description:
We are seeking a compassionate and skilled Occupational Therapist (OT) to provide rehabilitative care within a skilled nursing facility setting. The OT will evaluate, plan, and deliver therapeutic interventions to support residents in achieving improved functional independence and quality of life.
Responsibilities:
Conduct comprehensive evaluations to determine patient needs and establish individualized treatment plans.
Provide therapeutic interventions targeting strength, mobility, coordination, cognition, and daily living skills.
Adapt treatment approaches based on patient progress, safety, and therapeutic goals.
Collaborate closely with interdisciplinary team members to ensure cohesive patient care.
Educate patients, families, and caregivers on exercises, adaptive techniques, and safety strategies.
Document assessments, progress notes, and treatment outcomes in accordance with professional and facility standards.
Recommend adaptive equipment and environmental modifications as needed.
Maintain a safe and supportive therapy environment.
Comply with all regulatory, ethical, and facility guidelines.
Promote patient-centered care with empathy, professionalism, and clinical expertise.
