Job Title: Speech-Language Pathologist (SLP)

Location: Windsor Skyline Care 
Shift: Day Shift (8:00 AM – 4:30 PM)
Contract Length: 14 Weeks

Job Summary

The Speech-Language Pathologist (SLP) provides diagnostic, therapeutic, and rehabilitative services to patients with communication, cognitive-linguistic, and swallowing disorders. This position supports residents within the Windsor Skyline Care facility, ensuring high-quality treatment plans and patient-centered care to improve functional outcomes.

Key Responsibilities

  • Conduct evaluations of speech, language, voice, cognition, and swallowing disorders.

  • Develop individualized treatment plans (ITPs) based on clinical assessments and patient goals.

  • Provide direct therapy for speech, language, cognitive, and swallowing impairments.

  • Document assessments, treatment sessions, progress notes, and discharge summaries in the EMR.

  • Collaborate with the interdisciplinary care team, including nursing, PT, OT, and physicians.

  • Educate patients, families, and caregivers on therapeutic techniques and home programs.

  • Participate in care plan meetings and provide professional recommendations.

  • Monitor patient progress and adjust plans of care as needed.

  • Ensure compliance with facility policies, state regulations, and ASHA standards.

  • Maintain a safe and therapeutic environment for all patients.

Required Skills & Qualifications

  • Master’s degree in Speech-Language Pathology.

  • Active state SLP license (or ability to obtain before start date).

  • Certificate of Clinical Competence (CCC-SLP) preferred; CFY may be considered based on facility needs.

  • Previous experience in a skilled nursing facility (SNF), rehabilitation setting, or long-term care preferred.

  • Strong knowledge of dysphagia management and cognitive-communication disorders.

  • Excellent documentation, communication, and clinical reasoning skills.

  • Ability to work independently and as part of an interdisciplinary team.

Preferred Qualifications

  • Experience with geriatric populations.

  • Familiarity with EMR systems used in rehabilitation settings.

  • Ability to manage a diverse caseload efficiently.
