Licensed Practical Nurse (LPN) – Night Shift

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) for a night-shift role in a skilled nursing environment. The LPN will provide high-quality resident care, administer medications, document clinical interventions, and support nursing assistants throughout the shift. This role requires professionalism, strong communication skills, and a commitment to maintaining resident safety, dignity, and rights.

Key Responsibilities

  • Administer medications and treatments safely, accurately, and in accordance with all applicable laws, policies, and procedures.

  • Document and communicate resident care and clinical changes to the interdisciplinary team, ensuring HIPAA compliance.

  • Perform routine assessments and complete required documentation, including incident reports and infection control reports.

  • Participate in QAPI activities as assigned to support quality improvement initiatives.

  • Supervise and support nursing assistants to ensure quality resident care and adherence to facility policies.

  • Assist in resolving workflow concerns, answering questions, and reinforcing proper care procedures during the shift.

  • Act as an advocate for residents, respecting their rights, preferences, and directives.

  • Maintain a professional and compassionate demeanor when interacting with residents, families, coworkers, and visitors.

  • Perform assessments and communicate findings to physicians when necessary.

  • Initiate, review, and update resident care plans.

  • Document all care provided in medical records accurately and in a timely manner.

  • Perform other duties as assigned by the supervising nurse or facility leadership.

Qualifications

  • Active Licensed Practical Nurse (LPN) license (state-specific).
