We are seeking a dedicated and experienced Physical Therapist (PT) for a contract opportunity in a skilled nursing facility. The PT will evaluate residents, create individualized treatment plans, and deliver rehabilitative services aimed at improving mobility, strength, and overall functional independence.
Conduct comprehensive physical therapy evaluations to determine functional limitations and treatment needs.
Develop and implement personalized treatment plans using evidence-based therapy techniques.
Provide therapeutic exercises, gait training, balance activities, and manual therapy interventions.
Document evaluations, progress notes, and treatment outcomes in accordance with professional and facility standards.
Collaborate with interdisciplinary team members to optimize patient care and outcomes.
Participate in discharge planning and make recommendations for continued therapy or equipment needs.
Educate residents and caregivers on mobility strategies, exercise programs, and safety techniques.
Maintain a safe and effective treatment environment.
Active Physical Therapist (PT) license in the state of California (or eligibility to obtain).
Experience in skilled nursing or geriatric rehabilitation preferred.
Strong communication, assessment, and clinical decision-making skills.
Ability to work independently and collaboratively within a multidisciplinary team.
