We are seeking an experienced and dedicated Physical Therapist (PT) for a contract opportunity within a skilled nursing facility setting. The PT will be responsible for evaluating patients, developing individualized treatment plans, and delivering high-quality rehabilitative care to support mobility, recovery, and overall functional improvement.
Perform comprehensive evaluations of patients’ physical abilities and functional limitations.
Develop and implement evidence-based treatment plans tailored to patient needs.
Provide therapeutic exercises, mobility training, and manual therapy interventions.
Document evaluations, progress notes, and treatment outcomes in accordance with professional standards.
Collaborate closely with interdisciplinary team members to coordinate patient care.
Assist with discharge planning and provide recommendations for continued therapy.
Educate patients and caregivers on safe mobility techniques and home exercise programs.
Adhere to all regulatory, ethical, and facility-specific guidelines.
Active state license as a Physical Therapist (PT).
Prior experience in a skilled nursing facility preferred.
Strong assessment, communication, and clinical decision-making skills.
Ability to work both independently and alongside a multidisciplinary team.
