Position Overview
We are seeking an experienced and patient-centered Speech-Language Pathologist (SLP) for a 13-week contract assignment supporting post-acute and long-term care residents. This role involves providing high-quality communication, cognitive, and swallowing interventions across two skilled nursing facilities.
Conduct comprehensive assessments for speech, language, cognition, swallowing, and communication disorders.
Develop and implement individualized treatment plans based on clinical findings.
Provide therapy sessions to support functional communication and safe swallowing.
Document all evaluations, treatments, and progress notes in accordance with regulatory and facility guidelines.
Collaborate with interdisciplinary care teams to support holistic patient outcomes.
Educate patients, families, and staff regarding communication strategies and dysphagia safety.
Support continuity of care across two assigned facility locations as needed.
Active and unencumbered SLP license in the state of practice.
Certificate of Clinical Competence (CCC-SLP) required.
Experience in skilled nursing, post-acute rehabilitation, or geriatrics preferred.
Strong clinical reasoning, communication, and documentation skills.
Ability to work independently while maintaining collaborative teamwork.
