Seaford Center is seeking a motivated and skilled Occupational Therapist (OT) for a 14-week assignment. The OT will provide high-quality rehabilitative services focused on helping patients regain independence, enhance functional abilities, and improve their overall quality of life within a supportive post-acute care environment.
Conduct comprehensive patient evaluations and develop individualized treatment plans.
Deliver evidence-based therapeutic interventions to improve ADLs, fine motor skills, strength, and functional mobility.
Monitor patient progress and modify care plans as clinically appropriate.
Accurately document all assessments, treatments, and patient responses according to facility standards.
Collaborate with interdisciplinary teams—including PT, SLP, nursing, and case management—to support patient goals.
Educate patients and families on therapeutic exercises, safety practices, and continued care strategies.
Participate in care planning, discharge recommendations, and team meetings as needed.
Maintain compliance with facility, state, and federal documentation and practice standards.
Active state Occupational Therapist (OT) license.
Graduate of an accredited OT program (OTD or MOT).
Experience in post-acute, SNF, or rehabilitation settings preferred.
Strong communication, clinical reasoning, and documentation abilities.
Ability to work both independently and collaboratively within a multidisciplinary team.
Current CPR/BLS certification.
