We are seeking a dedicated Physical Therapist (PT) for a 13-week assignment in a post-acute/rehabilitation setting. The ideal candidate will provide high-quality therapy services focused on patient mobility, recovery, and functional independence. This role is suited for a PT who excels in patient-centered care and thrives in a collaborative clinical environment.
Evaluate patients and develop individualized treatment plans aligned with clinical standards and therapy goals.
Provide therapeutic interventions to improve mobility, strength, and functional performance.
Document assessments, treatment sessions, and progress notes in accordance with professional and regulatory requirements.
Collaborate with interdisciplinary care teams to coordinate and optimize patient care plans.
Educate patients and families on therapy processes, home exercises, and recovery expectations.
Maintain high clinical standards and contribute to a supportive and efficient therapy environment.
Active and unrestricted Physical Therapist (PT) license in the state of practice.
Previous experience in post-acute care, inpatient rehab, or skilled nursing settings preferred.
Strong clinical reasoning, patient communication, and documentation skills.
Ability to work independently and as part of a multidisciplinary team.
