Position: Travel LPN (T-LPN)
Shift: Evening Shift (2:45 PM – 11:15 PM)
Contract Duration: 16 weeks
Schedule: Sunday, Monday, Tuesday, Friday, Saturday
The Travel Licensed Practical Nurse will provide skilled nursing care in a long-term care/rehabilitation setting. This role supports resident care, medication administration, and coordination with the interdisciplinary team to ensure high-quality outcomes.
Provide direct nursing care in accordance with established care plans and facility policies.
Administer medications and treatments safely and accurately.
Monitor patient health, report changes, and document care in compliance with regulatory standards.
Collaborate with RNs, CNAs, and other healthcare staff to maintain a safe and supportive environment.
Assist with admissions, discharges, and transfers as needed.
Maintain accurate charting and ensure adherence to HIPAA and state nursing regulations.
Communicate effectively with residents, families, and the clinical team.
Active and unencumbered LPN license.
Previous experience in skilled nursing, long-term care, or rehabilitation setting preferred.
Strong clinical assessment and medication administration skills.
Ability to work evening shifts and a varied weekly schedule.
Excellent communication, teamwork, and documentation skills.
