Position: Licensed Practical Nurse (LPN)
Shift: Evening Shift (03:00 PM – 11:30 PM)
Shift Length: 8 hours
Contract Duration: 8 weeks
The Licensed Practical Nurse (LPN) will provide high-quality nursing care in a fast-paced skilled nursing or long-term care setting. The clinician ensures safe patient care, supports RN staff, and adheres to clinical and regulatory standards while delivering compassionate, resident-centered services.
Provide direct patient care including medication administration, treatments, and wound care as delegated.
Monitor residents’ health status, vital signs, and clinical changes; promptly report concerns to supervising RNs or providers.
Assist with admissions, discharges, and ongoing care planning.
Document assessments, interventions, and outcomes in accordance with facility and regulatory requirements.
Collaborate with RNs, CNAs, therapists, and interdisciplinary team members to support comprehensive patient care.
Maintain accurate medication records and follow safe medication administration practices.
Support residents with ADLs as needed, ensuring comfort, dignity, and safety.
Participate in shift handoffs, team meetings, and quality improvement activities.
Active state licensure as a Licensed Practical Nurse (LPN).
Experience in skilled nursing, long-term care, or rehabilitation settings preferred.
Strong clinical judgment, communication, and documentation skills.
Ability to work independently while collaborating effectively with the care team.
Demonstrated commitment to providing compassionate and ethical patient care.
