Job Title: Part-Time Social Worker (Wed–Fri)
Location: New York, NY 10011
Schedule: 24 hrs/week (Wednesday–Friday, 9:00 AM–5:00 PM)
Contract Type: Part-Time Contract
Pay: $42/hour
Overview:
"Navitas Healthcare, LLC" is seeking a compassionate Licensed Master Social Worker (LMSW) to support patients and families in addressing social, emotional, and financial challenges related to healthcare needs. This role involves assessment, counseling, discharge planning, and coordination with community resources.
Responsibilities:
Conduct psychosocial assessments and develop care plans.
Provide counseling and connect patients to community services.
Coordinate discharge and referrals for continued care.
Collaborate with healthcare teams for integrated care.
Maintain accurate documentation and participate in quality initiatives.
Qualifications:
Master’s Degree in Social Work (MSW) required.
Active LMSW license (LCSW preferred).
Healthcare or medical social work experience a plus.
Strong communication, problem-solving, and organizational skills.
Proficient in MS Office and EHR systems.
