Position Title: Social Worker – LCSW (Behavioral Health / Emergency Department)
Location: Hyannis, MA 02601
Job Type: Contract
Duration: 13 Weeks
Schedule: 24 hours per week, rotating evenings (3:00 PM – 11:00 PM or 4:00 PM – 12:00 AM), every other weekend, rotating holidays in the ED
Pay Rate: $50 – $56 per hour
"Navitas Healthcare, LLC" is seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to provide behavioral health assessments, crisis intervention, and discharge planning within an acute care and emergency department setting. The ideal candidate will work collaboratively with a multidisciplinary team to ensure continuity of care and optimal patient outcomes.
Conduct comprehensive clinical assessments for individuals presenting with acute care needs in compliance with state and departmental standards.
Develop and communicate initial treatment and discharge plans in collaboration with psychiatrists and interdisciplinary teams.
Facilitate individual, family, and group counseling sessions.
Coordinate care planning to ensure a safe and effective transition to home or community-based services.
Evaluate psychosocial needs and connect patients to appropriate community resources and programs.
Assist patients and families in decision-making related to placement or facility-based care as needed.
Provide crisis intervention and post-discharge support to patients and caregivers.
Maintain accurate and timely documentation for managed care authorization and regulatory compliance.
Serve as a liaison between patients, families, and community agencies to promote access to ongoing support.
Support community outreach and education initiatives related to behavioral health awareness.
Education: Master’s degree in Social Work from an accredited institution.
Licensure: Active Massachusetts Licensed Clinical Social Worker (LCSW) certification required.
Certifications:
Crisis Prevention Intervention (CPI)
Basic Life Support (BLS)
Skills: Strong clinical assessment, communication, and crisis management abilities. Experience in behavioral health or emergency settings preferred.
