Job Title: Travel Registered Nurse – Case Manager
Location: Hyannis, MA
Pay: $2,761/week
Shift: Rotating Days, 8:00 AM – 4:30 PM
Schedule: Every other weekend, rotating holidays

Position Overview:

Navitas Healthcare, LLC is seeking Registered Nurse – Case Manager for an exciting Travel or Local job in Hyannis, MA.

Key Responsibilities:

  • Screen new admissions daily to assess discharge planning needs and prioritize visits
  • Verify and update patient demographics and insurance information
  • Complete initial and ongoing assessments using clinical guidelines and utilization criteria
  • Evaluate appropriateness of admission and level of care; initiate discharge notices and appeals as necessary
  • Identify patients with complex psychosocial or legal needs and coordinate appropriate support services
  • Facilitate timely utilization review and collaborate with third-party payers
  • Work closely with physicians and interdisciplinary teams to develop and implement care and discharge plans
  • Advocate for patient needs, especially concerning insurance coverage and care decisions
  • Maintain accurate documentation in compliance with regulatory and organizational standards
  • Participate in care conferences and quality assurance initiatives

Qualifications:

  • Minimum of 3 years recent experience in acute care clinical or case management (within the past 5 years)
  • Proficient in the use of clinical decision-support tools such as InterQual® or similar systems
  • Recent discharge planning and utilization review experience (within the last 4 years)
  • Active Registered Nurse (RN) license in the state of Massachusetts
  • Bachelor of Science in Nursing (BSN) preferred
  • Certification in Case Management (e.g., CCM, CPUM) preferred
  • Strong communication, interpersonal, and negotiation skills
  • Ability to work independently and manage multiple priorities in a fast-paced environment
  • Proficient in computer applications and electronic documentation systems
