Job Title: RN - Home Health Nurse

Travel Rate: $2,498.50/Week
Schedule: Monday–Friday | Occasional Weekends & Holidays
We are seeking a compassionate and skilled Home Health Nurse to provide comprehensive assessments and deliver high-quality nursing care in a home setting. This role plays a key part in ensuring patient safety, comfort, and well-being through individualized care plans and collaboration with an interdisciplinary healthcare team.
Perform thorough nursing assessments during patient admissions, considering physical, psychosocial, environmental, and health history factors.
Develop, implement, and update individualized care plans to meet case management objectives.
Maintain Home Health Resource Group (HHRG) values at national standards and utilize them to establish visit frequency and interdisciplinary care.
Actively participate in patient case conferences and evaluate the effectiveness of care plans.
Supervise and provide instruction to Home Health Aides at least every 14 days.
Communicate promptly with physicians and care providers regarding significant changes in patient condition.
Complete all admission, discharge, and routine documentation accurately and in a timely manner.
Document skilled nursing care with clear, concise narratives covering teaching, direct care, and patient/caregiver responses.
Obtain, verify, and clarify valid physician orders for all care provided.
Plan and coordinate patient discharges, ensuring effective communication with both patients and physicians.
Perform other related duties as assigned.
Current Registered Nurse (RN) license in Massachusetts (required).
Successful completion of a pre-hire medication test.
Experience: Minimum 1 year of nursing experience, or 6 months with a BSN degree.
Certifications: CPR certification required.
Preferred: Prior home care and IV experience, BSN degree, and proficiency with computer-based documentation.
