Travel Rate: $3,248.50/Week
We are seeking an experienced Registered Nurse (RN) to serve as a Home Health Clinical Supervisor. In this role, you will oversee and coordinate the clinical operations of the home health team, ensuring compliance with all regulatory standards while promoting high-quality patient care. This leadership position requires strong clinical expertise, effective communication, and the ability to guide a multidisciplinary team.
40 hours per week, Monday–Friday
Occasional weekend and holiday hours required
On-call responsibilities included
Supervise daily clinical operations and staff activities in alignment with federal, state, and local regulations.
Collaborate with physicians, discharge planners, and community partners to optimize patient care and expand program awareness.
Act as a liaison between physician offices, skilled nursing facilities, and home health staff to coordinate services.
Oversee patient care management and safety within the designated service area.
Support hiring, orientation, training, coaching, and evaluation of staff in partnership with Human Resources.
Conduct team meetings, participate in interdisciplinary care conferences, and lead case reviews.
Provide guidance and clinical expertise to staff, including Certified Home Health Aides (CHHAs).
Rotate through clinical on-call management, ensuring patient and staff safety during weekends and holidays.
Deliver direct patient care when necessary in accordance with agency requirements.
Ensure adherence to Department of Public Health (DPH), grant, accreditation, and other regulatory standards.
Maintain confidentiality and uphold excellent customer service.
Perform additional duties as assigned by management.
Active Registered Nurse (RN) license in Massachusetts (required).
Minimum of 5 years recent clinical nursing experience, with at least 2 years in home care or community health (home health strongly preferred).
Demonstrated leadership, organizational, and communication skills.
Proficiency with computers and electronic health record systems preferred.
