Position Overview

We are seeking an experienced Registered Nurse (RN) to serve as a Home Health Clinical Supervisor. In this role, you will oversee and coordinate the clinical operations of the home health team, ensuring compliance with all regulatory standards while promoting high-quality patient care. This leadership position requires strong clinical expertise, effective communication, and the ability to guide a multidisciplinary team.

Schedule Details

  • 40 hours per week, Monday–Friday

  • Occasional weekend and holiday hours required

  • On-call responsibilities included

Responsibilities

  • Supervise daily clinical operations and staff activities in alignment with federal, state, and local regulations.

  • Collaborate with physicians, discharge planners, and community partners to optimize patient care and expand program awareness.

  • Act as a liaison between physician offices, skilled nursing facilities, and home health staff to coordinate services.

  • Oversee patient care management and safety within the designated service area.

  • Support hiring, orientation, training, coaching, and evaluation of staff in partnership with Human Resources.

  • Conduct team meetings, participate in interdisciplinary care conferences, and lead case reviews.

  • Provide guidance and clinical expertise to staff, including Certified Home Health Aides (CHHAs).

  • Rotate through clinical on-call management, ensuring patient and staff safety during weekends and holidays.

  • Deliver direct patient care when necessary in accordance with agency requirements.

  • Ensure adherence to Department of Public Health (DPH), grant, accreditation, and other regulatory standards.

  • Maintain confidentiality and uphold excellent customer service.

  • Perform additional duties as assigned by management.

Qualifications

  • Active Registered Nurse (RN) license in Massachusetts (required).

  • Minimum of 5 years recent clinical nursing experience, with at least 2 years in home care or community health (home health strongly preferred).

  • Demonstrated leadership, organizational, and communication skills.

  • Proficiency with computers and electronic health record systems preferred.
