Position: Licensed Practical Nurse (LPN) – Night Shift (PRN / As Needed)
Location: Flagstaff AZ
Schedule: 12-hour shifts, 6:00 PM to 6:30 AM
Hours: Must work minimum of 3 shifts per month; 40 hours per week not guaranteed

Pay rate: $44/Hour
Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide patient-centered nursing care during night shifts. The LPN will administer medication, perform treatments, observe and report patient status, and support the interdisciplinary healthcare team within scope of nursing license and healthcare regulations.
Key Responsibilities:
• Administer medications and treatments as ordered by physicians, including various forms such as oral, enteral, parenteral, suppositories, sterile wound care, tracheostomy care, gastrostomy, jejunostomy care, and catheter care
• Observe and report on the effectiveness of medications and patient responses
• Ensure availability and proper use of supplies and medications
• Communicate medical, social, and functional needs of residents to the healthcare team, families, and residents
• Participate in shift report handoffs and complete initial nursing assessments on newly admitted residents
• Assist with MDS assessments, quarterly reviews, and development of individualized care plans
• Maintain thorough clinical and administrative documentation
• Participate in Quality Assurance and Performance Improvement (QAPI) programs and staff meetings
• Apply gerontological nursing principles and comply with federal, state, and facility regulations
• Monitor residents consistently and respond appropriately to medical emergencies
• Establish effective working relationships with residents, families, and team members
• Support and supervise clinical care assistants and other support staff as needed
Qualifications:
• Current valid LPN license
• Knowledge of nursing principles, pharmacology, patient care, and gerontology
• Excellent observational, communication, and critical thinking skills
• Ability to work independently and as part of a multidisciplinary team
• Strong interpersonal skills and commitment to compassionate patient care
