Position Overview

We are seeking an experienced Home Health Clinical Supervisor (RN) to oversee home health operations and ensure high-quality patient care in compliance with state, federal, and accreditation standards. The ideal candidate will provide clinical leadership, staff support, and patient safety oversight while fostering collaborative relationships across healthcare teams.

Responsibilities

  • Maintain effective communication with physicians, discharge planners, and healthcare partners.

  • Act as a liaison between physician offices, skilled nursing facilities, and the home health team.

  • Coordinate services for patients transitioning from inpatient to home care.

  • Develop and enforce evidence-based practice standards.

  • Partner with HR to support hiring, onboarding, and orientation processes.

  • Coach, mentor, and monitor performance of direct reports.

  • Lead and participate in care conferences, case reviews, and team meetings.

  • Provide clinical expertise and serve as a resource for staff and the community.

  • Rotate through clinical management on-call duties and ensure coverage for weekends and holidays.

  • Deliver direct patient care when necessary.

  • Ensure compliance with all Department of Public Health (DPH), governmental, grant, and JCAHO standards.

  • Demonstrate consistent service excellence to patients, families, and staff.

Qualifications

  • Active Registered Nurse (RN) license in Massachusetts (required).

  • Minimum of 5 years of recent clinical experience, including at least 2 years in home health or community health (home health strongly preferred).

  • Strong organizational, leadership, and communication skills.

  • Proficiency with clinical and administrative software systems preferred.
