Job Title: Licensed Master Social Worker (LMSW)
Location: New York, NY 10016
Pay Rate: $2,062.50 – $2,250.00 per week
Schedule:
Hours per Day: 8
Days per Week: 5
Total Hours per Week: 40
Shift Time: 9:00 AM – 5:00 PM
Key Responsibilities:
Perform psychosocial assessments and develop individualized care plans
Support discharge planning by coordinating with community resources and social services
Deliver crisis intervention, emotional support, and short-term counseling
Work collaboratively with physicians, nurses, case managers, and allied health professionals
Advocate for patients and assist in addressing social, emotional, and financial barriers to care
Maintain thorough and timely documentation in electronic medical records
Participate in interdisciplinary rounds and case conferences
Qualifications:
Active LMSW license issued by the New York State Education Department (NYSED) (primary source verification required)
Minimum 1 year of experience practicing as an LMSW
Recent hospital or inpatient experience strongly preferred
Proficient in discharge planning, community referrals, and social service navigation
Excellent interpersonal, counseling, and problem-solving skills
Ability to function both independently and within a multidisciplinary care team
