Job Title: Travel Registered Nurse (RN) – Case Management
Location: Hyannis, MA
Department: Case Management
Shift: 8:00 AM – 4:30 PM | 40 hours/week
Schedule: Every other weekend | Rotating holidays
Pay Rate: $80.03/hour | $3,201/week

Position Summary:

We are seeking an experienced and detail-oriented Travel RN – Case Manager to support care coordination and utilization review in an acute care setting. The ideal candidate will demonstrate strong clinical knowledge, exceptional communication skills, and a deep understanding of discharge planning and payer guidelines. This is a travel contract position located in Hyannis, Massachusetts.

Key Responsibilities:

  • Conduct daily discharge planning screens and establish case prioritization

  • Complete InterQual® Initial Assessments within one business day and perform reassessments every three days or as clinically indicated

  • Determine appropriate level of care (Inpatient vs. Observation)

  • Issue HINN notices, facilitate appeal processes, and manage reinstatement letters

  • Perform utilization reviews and liaise with third-party payers

  • Coordinate discharge and care plans in collaboration with interdisciplinary teams

  • Advocate for patients during payer communications and discharge planning

  • Participate in daily unit rounds, family meetings, and care conferences

  • Facilitate safe discharge transitions, referrals, and community resource linkage

  • Utilize and update Care Maps or clinical pathways to guide care

  • Identify delays in treatment and collaborate to reduce length of stay

  • Assist coding and documentation teams with clarifying clinical information

  • Maintain current knowledge of regulatory policies and community resources

Required Qualifications:

  • Current RN license in the Commonwealth of Massachusetts

  • Minimum 3 years of acute care experience within the past 5 years in a hospital setting

  • Recent experience (within the past 4 years) in Discharge Planning and Utilization Review

  • Strong understanding of InterQual® or equivalent criteria system

  • Broad clinical background

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) preferred (required for external applicants)

  • Certification in Case Management, CPUM, or a specialty clinical area
