Position: Case Manager – RN

Location: Houston, TX

Shift: 8:00 AM – 4:30 PM

Duration: 13 Weeks with possible extension

Schedule: Must be available to join the holiday rotation if applicable

Job Summary:

Seeking a skilled Registered Nurse Case Manager to support and coordinate inpatient case management operations. This role involves overseeing care facilitation, utilization management, discharge planning, and quality improvement initiatives to ensure efficient and patient-centered care.

Responsibilities:

• Support implementation and operations of the inpatient case management program
• Oversee daily departmental functions and care coordination processes
• Manage hospital resource utilization effectively
• Collaborate with interdisciplinary teams on discharge planning and quality care initiatives
• Participate in data collection and analysis for performance and quality improvement
• Ensure compliance with clinical and operational standards

Requirements:

• Active RN license (TX or compact)
• Recent experience in inpatient case management
• Strong background in discharge planning and utilization review
• Excellent communication and coordination skills
• Willingness to adhere to all facility policies and schedule requirements
