Job Title: Case Manager – LPN/RN (Emergency Department)

Duration: 13 Weeks contract

Location: Hyannis, MA

Shift:- 11:00 AM – 9:30 PM or 11:30 AM – 10:00 PM | 40 hours/week | Four (4) 10.5-hour shifts

Pay:

Travel: $2,900 weekly include stipend

REQUIREMENT: MA license, BLS

Job Duties:

  • Conduct daily screening of new admissions to assess discharge planning needs.
  • Verify demographic and payer information; notify admissions of necessary corrections.
  • Review and evaluate admission appropriateness and level of care (Inpatient vs. Observation).
  • Identify and refer high-risk patients with complex psychosocial, financial, or legal needs.
  • Facilitate communication with third-party payers to support concurrent clinical information requests.
  • Participate in and facilitate daily rounds; collaborate with physicians and care team on care and discharge planning.
  • Act as a patient advocate, providing information regarding insurance coverage and discharge planning.
  • Coordinate referrals and ensure complete and accurate transfer of information to external facilities or services.
  • Apply knowledge of community resources and act as a resource to care teams.
  • Evaluate and monitor patient progress through clinical pathways and Care Maps.
  • Work with medical coders to ensure accurate diagnosis and procedure coding.
  • Maintain compliance with regulatory requirements, infection control, and safety standards.
  • Stay updated on current trends in case management and payer regulations through ongoing education.
  • Display initiative, independence, and strong time management in daily responsibilities.

Required Qualifications:

  • Current RN or LPN license in the Commonwealth of Massachusetts
  • Minimum 3 years of acute care experience in the past 5 years in a hospital or case management setting
  • BLS Certification (required)
  • Strong organizational, communication, and negotiation skills
  • Ability to manage multiple priorities in a fast-paced and high-stress environment
  • Proficient in computer systems and clinical documentation software
