Job Title: Registered Nurse – Case Manager
Duration: 13 Weeks Contract
Location: Bronx, NY
Shift: Monday – Friday | 9:00 AM – 5:00 PM | Full-Time (40 hours/week)
Active New York State RN License (Primary Source Verification required)
Basic Life Support (BLS) certification from the American Heart Association (AHA)
Perform comprehensive case and utilization management assessments to ensure effective care planning and timely discharges
Collaborate with interdisciplinary teams to coordinate patient care across the healthcare continuum
Track patient progress and resource use to support quality clinical outcomes
Accurately document all case management activities in compliance with regulatory and organizational standards
Serve as a liaison between patients, families, and healthcare providers to streamline care transitions
Utilize EPIC for managing and documenting case management workflows
Active New York State Registered Nurse license
BLS certification (AHA)
Minimum of 2 years’ experience in case/resource management and utilization review
