Position Overview:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our night shift team at a reputable skilled nursing and residential care facility in Prescott, Arizona. Key Responsibilities:
Administer medications and treatments safely and in accordance with all applicable policies and regulations.
Document care provided, patient condition, and treatment outcomes accurately in medical records.
Monitor residents’ health and report significant changes to the interdisciplinary team.
Collaborate with physicians and healthcare team members to ensure continuity of care.
Supervise nursing assistants during shift and ensure quality care delivery.
Respond to medical emergencies and perform assessments as needed.
Participate in infection control protocols and quality improvement initiatives.
Maintain confidentiality and adhere to HIPAA guidelines.
Support residents’ rights and advocate for patient-centered care.
Perform additional duties as assigned by nursing leadership.
Requirements:
Active LPN license in the state of Arizona or eligibility for licensure.
Ability to work independently and as part of a team in a fast-paced environment.
Strong clinical and documentation skills.
Excellent interpersonal and communication abilities.
Commitment to providing compassionate, high-quality care.
Preferred:
Previous experience in a skilled nursing or long-term care setting.
