Requirements:

  • Current or temporary Arizona LPN license

  • High school diploma or equivalent

  • Current Level 1 Fingerprint Clearance Card (AZ DPS)

  • CPR/BLS certification obtained through physical assessment

Job Description: We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) for a 13-week assignment in Phoenix, AZ. This role offers both day and night shift options with alternating weekend coverage. Ideal candidates are detail-oriented, team-driven, and committed to high-quality patient care.

Responsibilities:

  • Administer prescribed medications and perform treatments as ordered by physicians and within licensure scope

  • Handle medications in various forms, including oral, enteral, parenteral, and suppository

  • Deliver treatments such as wound care, tracheostomy and catheter maintenance, gastrostomy/jejunostomy care, and postural drainage

  • Monitor, observe, and report medication and treatment effectiveness

  • Ensure adequate availability of medical supplies and medications

  • Collaborate with residents, families, and staff to support comprehensive, interdisciplinary care

  • Complete initial nursing assessments upon resident admission

  • Maintain accurate clinical and administrative documentation regarding diagnoses, treatments, observations, and patient progress

  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and Arizona Management System (AMS) practices

  • Provide in-service training and orientation for staff and new hires

Knowledge, Skills & Abilities:

  • Strong foundation in geriatric and practical nursing theory and techniques

  • Understanding of medical, physical, and psychological changes in elderly populations

  • Ability to assess, observe, and document patient behavior and progress

  • Knowledge of state laws and regulations related to long-term care

  • Skilled in contributing to team-based care plans and nursing strategies
