Requirements:

  • Active Certified Nursing Assistant (CNA) certification in the State of Arizona

  • Current Level 1 Fingerprint Clearance Card

  • CPR/BLS certification

  • COVID-19 vaccination or signed declination

Job Description: We are seeking compassionate and dependable Certified Nursing Assistants (CNAs) for a 13-week assignment in Phoenix, AZ. This role involves providing direct care and support to residents across both day and night shifts, with alternating weekend coverage.

Responsibilities:

  • Assist residents with activities of daily living (ADLs), including transfers, ambulation, bathing, grooming, dressing, toileting, and feeding

  • Provide emotional and physical support to promote resident dignity and independence

  • Monitor and report changes in residents’ physical and mental health status; record vital signs accurately

  • Assist with basic medical procedures, specimen collection, and both post-surgical and post-mortem care

  • Support residents during mealtimes, including menu guidance and physical assistance

  • Maintain clean and safe living environments; change linens and support hygiene protocols

  • Respond promptly to call lights and resident needs, ensuring care aligns with the Resident Bill of Rights

  • Document care provided and any observations in medical records or assigned worksheets

  • Help transport residents to offsite appointments and perform basic clerical duties as required

  • Participate in Arizona Management System (AMS) initiatives such as Huddle Boards, Job Breakdown Sheets, and Standardized Work

  • Contribute to Quality Assurance and Performance Improvement (QAPI) initiatives as outlined by CMS
