Shift: Monday – Friday | 8:30 AM – 5:00 PM | 40 Hours/Week
Contract Duration: 2+ Months

Compensation: $1,500 – $2,000 per week

Job Summary:

A well-established healthcare organization is seeking a dedicated Registered Nurse Case Manager to provide comprehensive case management and nursing support during the prenatal and postpartum periods. The ideal candidate will offer compassionate, evidence-based care, perform client screenings, provide education, and ensure coordinated access to community resources.

Key Responsibilities:

  • Deliver equitable nursing case management services for prenatal, postpartum, and infant care populations.

  • Conduct screenings for Social Determinants of Health (SDOH), perinatal depression, and other prenatal/postpartum risk factors at admission and as needed.

  • Document all assessments and interactions in the Electronic Medical Record (EMR) in a timely and accurate manner.

  • Offer client education and guidance in line with current perinatal care standards.

  • Provide short-term interventions, emotional support, and connect clients with appropriate community resources.

  • Develop personalized care plans and track client progress through measurable goals.

  • Respond to all communication (email, phone, or text) within one business day.

  • Conduct postpartum home visits within 30 days of delivery as needed.

  • Maintain accurate program data and submit monthly statistics to supervisor by the 5th of each month.

  • Participate in required virtual meetings, workshops, and training sessions.

Qualifications & Certifications (Required Prior to Start):

  • Active Registered Nurse (RN) License (State of Maryland or Compact)

  • Minimum 1 year of relevant nursing/case management experience

  • BLS Certification (AHA)

  • Strong documentation and communication skills

  • Experience with EMR systems preferred

  • Compassionate, resourceful, and committed to maternal-child health
