Registered Nurse – Transitional Care Case Manager

Qualifications:

• Active RN license with experience in case management and care transitions.
• Minimum of 2 years of experience in hospital discharge planning or post-acute care.
• Strong knowledge of rehabilitation, home healthcare, and long-term care options.
• Experience with patient advocacy and healthcare navigation.
• Certification in Case Management (preferred).

Duties:

• Assist patients in transitioning from hospital to home or rehab facilities.
• Coordinate follow-up appointments and home health services.
• Provide education on post-discharge care and medication management.
• Monitor patient recovery and prevent avoidable readmissions.
• Collaborate with families, caregivers, and healthcare providers.
• Ensure insurance approvals and access to necessary medical services.
• Document patient interactions and progress reports.

Pay:

• Medium weekly pay range: $1,900 - $2,400
