Description:

The RN Long Term Care Case Manager provides oversight and coordination of care for patients in long-term care or skilled nursing settings. This nurse will assess patient needs, develop care plans, and ensure that patients receive the appropriate services. The case manager will also serve as a liaison between families, patients, and healthcare providers to optimize care outcomes.

Mandatory Skills:

• Registered Nurse (RN) with experience in long-term care or case management.
• Skilled in creating and managing care plans for patients with chronic and complex needs.
• Strong ability to assess patient conditions and recommend interventions.
• Proficiency in communication and collaboration with interdisciplinary teams.
• Experience in patient advocacy and coordinating family meetings.
